LifeQuest Team Registration Form

How to register your team for league play at LifeQuest fitness Center:

1. Team Captain's will fill out the Team Registration form, pay the registration fees and submit a roster of players. Team rosters may be submitted after registration, but must be within 10 days of the start of games. League deadlines will be posted on the website and on the calendar in the lobby at LifeQuest. www.lifequestfitness.net
2. Team Registration Fee: $350.00 early bird, $400 late Reg. The fee will include up to 14 players to be on the roster! All players who are not members of The Wellness Clinic at LifeQuest, and are over the age of 18 are required to purchase a season Soccer House Membership card. The card fee is $10.00. The season starts and ends in September of each year.
3. Team Registration Fee for Youth Division will include one complimentary field rental.
4. All rosters turned in will be considered final and will not be able to change until the following season. Players are not allowed to change teams during the current session.
5. Spectators over the age of 18 must sign a liability waiver and purchase a Soccer House membership card or pay a daily entry fee of $1 per day prior to entering the Soccer House.

Washington State Tax is included in all team fees.

In consideration of being allowed to participate in any way in the UfeQuest Health systems LLC/athletic sports programs, related events and activities, the

undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISK, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS LifeQuest, LLC/their members, officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners, and lessors of premises used to conduct the event (RELEASEES), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE.
4. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK OF AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRA TION. This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child's involvement or participation in these programs as provides above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.
